The Great Diabetic Paradox

Despite unprecedented access to medical knowledge and healthcare resources, the prevalence of diabetes continues to rise worldwide. The 5Plus2 Challenge views Type 2 diabetes not merely as a medical condition, but as a social illness, shaped by our culture, behaviors, and shared environments. Preventing it requires more than clinical treatment; it demands community engagement, cross-sector collaboration, and a shift toward prevention-centered living.

To address this issue, we have developed a community-based 4-pillar system. Together, they provide the framework that guides our efforts to prevent diabetes and delay the average age of onset through sustained awareness, lifestyle changes, and community action.

The 5 Pillars

The 5Plus2 Challenge is built upon five interconnected pillars, each designed to support a shared vision of diabetes prevention through awareness, engagement, and action:

  • Community Outreach — Local partnerships and volunteer-driven initiatives that bring prevention efforts into neighborhoods, workplaces, schools, and community organizations.
  • The Diabetes Support Network — A community-based support system that connects individuals with workshops, support group meetings, educational resources, and local care providers.
  • The Website — A centralized information hub that connects communities with educational tools, research, local resources, and support networks.
  • The 5Plus2 Challenge Week — A community-centered initiative that encourages individuals and groups to adopt practical lifestyle changes together through shared participation and accountability.
  • The Prevention Fair — A public event that showcases community resources, educational opportunities, success stories, health screenings, and collaborative partnerships focused on diabetes prevention.

Together, these five pillars form a sustainable framework for increasing awareness, strengthening collaboration, and empowering communities to take proactive steps toward preventing diabetes before it starts.

Community Outreach

Community Outreach Area Coordinators serve as vital ambassadors of the 5Plus2 Challenge within their assigned regions. With strong interpersonal skills and a passion for public health, COACs play a central role in expanding the reach of the organization by building local relationships, promoting awareness, and fostering active community participation in diabetes prevention.

Key Responsibilities

  • Engage actively with local clinics, hospitals, pharmacies, schools, faith-based institutions, and community centers to introduce and promote the 5Plus2 Challenge.
  • Represent the organization at health fairs, local medical exhibitions, and community wellness events.
  • Organize and manage information booths or tables at public venues such as malls, high schools, gyms, parks, and farmers’ markets.
  • Collaborate with volunteers and local partners to maximize outreach efforts and ensure consistent messaging across all activities.

Report regularly to the Community Outreach Team Leader (COTL) to share updates, feedback, and opportunities for improvement.

Diabetes Support Network

The Diabetes Support Network is a community-based initiative designed to connect individuals living with diabetes, or at risk of developing it, with ongoing support, educational workshops, support group meetings, and local health resources. Its goal is to foster encouragement, accountability, and community engagement while helping individuals make sustainable lifestyle changes and better manage their health.

The 5Plus2 Challenge Website

The 5Plus2 Challenge website serves as the central hub connecting every part of our mission — from individual participation to community outreach and partner collaboration. It is the platform where ideas, people, and resources come together to advance our shared goal of preventing diabetes through awareness and lifestyle change.

Designed as a go-to resource for individuals, families, and organizations, the website offers clear, actionable information about diabetes prevention, nutrition, physical activity, and community programs. Visitors can learn how to participate in the 5Plus2 Challenge Week, register for the Prevention Fair, or access local support through our Community Outreach Area Coordinators (COACs).

The 5Plus2 Challenge Week

At the heart of the 5Plus2 Challenge is a twice-yearly, one-week event held every spring and fall, dedicated to raising awareness and inspiring healthier habits through simple, collective action.

  • Monday to Friday:Participants commit to living a diabetes-friendly lifestyle, focusing on balanced meals, reduced sugar intake, and regular physical activity.
  • Saturday and/or Sunday – The Prevention Fair: The week concludes with community events that encourage participants to continue their healthy habits, celebrate progress, and connect with others.

In larger cities, an exhibition hall or community center hosts The Prevention Fair, featuring local and national vendors, wellness screenings, Q&A sessions with healthcare professionals, and activities such as 5K or 10K walks.

In smaller towns and neighborhoods, scaled-down local events are organized by Community Outreach Area Coordinators (COACs) and volunteers.

The Prevention Fair

The Prevention Fair is the highlight of every 5Plus2 Challenge Week, bringing communities together to celebrate health, learning, and shared progress. Designed as an interactive, community-centered event, the fair serves as a bridge between awareness and action transforming the message of prevention into an engaging, real-world experience.

Participants, families, and community members gather in an open, welcoming environment to explore resources, meet local and national vendors, and engage directly with health professionals. The fair often features wellness screenings, healthy food demonstrations, educational booths, and Q&A sessions with doctors, nutritionists, and fitness experts. It’s also a time for community fun with 5K or 10K walks, family activities, and interactive exhibits that make prevention both practical and enjoyable.
